Vasectomy Reversal procedures are used to reconnect the tubes (Vas Deferens) by bypassing the scarred area from a previous vasectomy, offering a chance to restore fertility despite the initial procedure being considered irreversible.
Vasovasostomy is the most common three-hour reversal technique, where the cut ends of the vas deferens are sewn back together using a high power surgical operating microscope and very fine sutures.
The choice between procedures depends on the presence of fluid: surgeons only perform a Vasovasostomy if fluid is still present in the testicular end of the Vas; if no fluid is present, they must perform an Epididymovasostomy.
Vasectomy Reversal is much less expensive than In Vitro Fertilization (IVF) and allows for multiple pregnancies, but a significant risk is failure (no sperm or low sperm count) due to scarring, congestion, or anti-sperm antibodies.

About Vasectomy Reversal Procedures

Although Vasectomy should be considered an irreversible procedure, this is not technically true. Changing social circumstances may lead to a new desire for fertility.

If a man has had a Vasectomy in the past, the tubes may be reconnected by bypassing the scarred area that is associated with the previous Vasectomy. There are various methods for performing a vasectomy reversal, and your surgeon will discuss the differences in each technique and which is most appropriate for your circumstances.

Two procedures are generally used for reverse vasectomies:

Vasovasostomy is the most common technique for vasectomy reversal.
During this three-hour procedure, the cut ends of the vas deferens are sewn back together using a high power surgical operating microscope and sutures finer than a human hair.

Anastomoses (reconnection of the previously severed tubes) are performed. Dissection of the Vas Deferens, above and below the obstructed segment from the previously performed Vasectomy is carried out, and then the placement of six to ten very fine sutures is made, to reconstruct an intact tube. Prior to placing any of the sutures our surgeons examine the fluid coming from the testicular end of the Vas under the microscope. Our surgeons only perform this procedure if fluid is still present in the Vas Deferens at the time of surgery; if no fluid is present, they perform an Epididymovasostomy.

Shortened: Vasovasostomy, a three-hour vasectomy reversal technique, reconnects the severed ends of the vas deferens using microsurgery and fine sutures. Surgeons first examine fluid from the testicular end under a microscope and proceed with vasovasostomy (placing six to ten sutures) only if fluid is present. If no fluid is found, they perform an epididymovasostomy.

Epididymovasostomy is a more complex version of the Vasovasostomy. This procedure is chosen if the blockage of sperm is at the level of the Epididymis. This procedure is one of the most complex microsurgical procedures performed.

This technique involves placing two or three sutures in the distended epididymal tubule and then connecting them to the vas deferens before opening the tubule. This makes the procedure significantly easier as the surgeon can place the sutures in the enlarged distended tubule rather than the open deflated tubule. Once all the sutures are placed, the tubule is opened with a microsurgical knife and the sutures are sequentially tied down.

Shortened: Epididymovasostomy, a more complex microsurgical version of vasovasostomy, is necessary when sperm blockage occurs in the epididymis. The technique involves placing two or three sutures in the distended epididymal tubule, connecting them to the vas deferens before opening the tubule. This simplifies the procedure by allowing suture placement in the enlarged tubule before it is opened and the sutures are tied.

  • Much less expensive than In Vitro Fertilization (IVF) or most reproductive technologies
  • Multiple pregnancies are possible after vasectomy reversal, unlike other reproductive technologies

  • No sperm or low sperm count, even if the Vas is reconnected; this can be the result of scarring, chronic congestion or the development of anti-sperm antibodies.
  • High risk of failure. If the vasectomy reversal fails, the patient will likely have to go on to IVF or an alternative reproductive technology.
  • Time interval since vasectomy does play a role. The quality and nature of Vasal fluid, and presence of Epididymal obstruction are also instrumental in the success of a Vasectomy Reversal.
