Distal Metatarsal Metaphyseal Osteotomy (DMMO) is a revolutionary, minimally invasive, percutaneous surgical technique used to treat severe forefoot pain, such as metatarsalgia (pain in the ball of the foot) and chronic calluses.
The procedure is performed through tiny 3mm skin incisions under X-ray guidance, utilizing a specialized cutting burr to make precise, angled cuts that shorten and slightly elevate the metatarsal bones to reduce under-foot pressure.
Unlike older techniques (such as the Weil Osteotomy), DMMO is hardware-free, using no internal screws or pins; instead, it relies on the body's natural alignment and intact ligaments to stabilize the bones and achieve uniform weight distribution.
Because the bone cuts are extra-articular (made completely outside the joint capsule), soft-tissue trauma is minimized, which preserves a better range of movement and reduces the risk of joint stiffness and scarring.
The recovery timeline allows for immediate weight-bearing right after surgery in a protective orthopaedic flat shoe; by Week 6, a follow-up X-ray is taken to confirm healing, allowing patients to discard the special shoe, resume driving, and transition to normal supportive footwear.

Distal Metatarsal Metaphyseal Osteotomy (DMMO) is a minimally invasive surgical procedure and a revolutionary, percutaneous technique used to alleviate severe forefoot pain, such as metatarsalgia (pain in the ball of the foot). By shortening and slightly elevating the bone, the pressure under the ball of the foot is reduced, which relieves pain and helps resolve calluses.

– Through tiny 3mm skin incisions, surgeons use a specialized, tiny cutting burr under X-ray guidance to make precise, angled cuts in the affected metatarsal bones to shorten and elevate the metatarsal bones.

– Unlike older procedures (like a Weil Osteotomy) which require screws or pins to hold the bone, DMMO relies on the body’s natural alignment and ligaments to stabilize the bones as they heal.

  • Smaller incisions mean less soft-tissue trauma and a reduced risk of scarring and joint stiffness.
  • Because of no internal hardware (like screws), the ligaments remain intact, allowing the bones to naturally adjust to a uniform weight distribution.
  • Immediate Weight-Bearing: Patients can put weight on the foot right after surgery, often walking in a protective surgical shoe.
  • Extra-articular: The bone cuts are made outside the joint capsule, which preserves a better range of movement.

  • Weeks 0-2: You will wear a thick post-operative bandage and a protective orthopaedic flat shoe, but you can walk as tolerated (often with the help of crutches).
  • Weeks 2-6: Bandages are routinely changed or checked, and swelling begins to reduce.
  • Week 6 & Beyond: A follow-up X-ray confirms the bones are healing in their new position. Patients can usually discard the special shoe, resume driving, and transition to firm, supportive footwear.
  • Months 3-6: Full recovery and resolution of swelling.
