Ear ventilation tubes (grommets) are tiny, hollow tubes surgically inserted into the eardrum to allow airflow into the middle ear and prevent fluid accumulation (a procedure known as a tympanostomy).
The outpatient procedure takes approximately 10 minutes under sedation or general anaesthesia (commonly in children, though local anaesthesia may be used for adults).
This treatment is highly beneficial for patients experiencing recurrent middle ear infections, persistent fluid (glue ear), related hearing loss, or children with delayed speech and language development linked to hearing difficulties.
During surgery, the specialist makes a small incision in the eardrum (myringotomy), suctions out trapped fluid, and positions the grommet, which usually falls out naturally within 9 to 18 months as the eardrum heals.

About Ear Ventilation Tubes (Grommets)

Ear ventilation tubes, commonly known as grommets, are tiny hollow tubes inserted into the eardrum to allow air to enter the middle ear and prevent fluid build-up. The procedure, known as tympanostomy tube insertion, is one of the most frequently performed ENT procedures in children, although it may also be recommended for adults.

The procedure is designed to improve middle ear ventilation, reduce recurrent ear infections and restore hearing that may be affected by persistent fluid behind the eardrum.

Ear ventilation tubes may be recommended for patients with:

  • Recurrent middle ear infections
  • Persistent fluid behind the eardrum (otitis media with effusion)
  • Hearing loss related to middle ear fluid
  • Children with Delayed speech or language development associated with hearing difficulties
  • Recurrent ear infections despite medical treatment
  • Eustachian tube dysfunction
  • Barotrauma related to pressure changes, particularly in adults

A thorough assessment, including hearing tests where appropriate, will help determine whether grommet insertion is the most suitable treatment option.

The procedure is performed under general anaesthesia in children and may be performed under local anaesthesia or sedation in selected adults.

The surgeon makes a small opening in the eardrum (myringotomy) and carefully removes any fluid trapped within the middle ear. A tiny ventilation tube is then inserted into the opening to allow ongoing airflow and drainage.

The tube remains in place while the eardrum heals around it, helping to equalise pressure and prevent fluid accumulation.

In some children, adenoidectomy may be recommended at the same time if enlarged adenoids are contributing to recurrent ear problems.

The procedure is quick, and most patients return home on the same day.

  • Scarring of the eardrum
  • Persistent perforation of the eardrum after the tube falls out
  • Recurrent ear infections
  • Need for repeat tube insertion

Serious complications are uncommon.

Recovery is usually rapid, with most patients returning to normal activities within 24 hours.

Following the procedure, patients may experience:

  • Mild ear discomfort

  • Small amounts of ear drainage

  • Attending follow-up appointments
  • Monitoring hearing improvement
  • Following guidance regarding swimming and water exposure

The tubes typically fall out naturally as the eardrum heals, usually within nine to eighteen months.

Regular follow-up appointments may be recommended to monitor tube function and middle ear health.
