The three main nerves of the hand are the Radial Nerve (extending wrist/fingers, sensation on back of hand), Ulnar Nerve (fine motor, sensation on little finger side), and Median Nerve (thumb/finger flexion, sensation on palm side).
Neurotmesis is the most severe nerve injury (completely severed nerve) that requires surgical intervention, which involves the surgeon reconnecting the nerve using a high-magnification microscope via End-to-End Repair, a Nerve Graft, or a Nerve Transfer.
To ensure a successful outcome, the procedure (typically 1 to 3 hours under general anesthesia) requires the nerve fibers to be precisely aligned under the microscope, followed by a comprehensive and progressive rehabilitation programme.

Nerve repair surgery of the hand is a complex and delicate procedure performed to reconnect torn or severed peripheral nerves. The nerves in the hand are essential for both sensory perception and motor control, enabling precise movement and function of the fingers and hand.

When these nerves are damaged—whether through trauma, compression, or laceration—they can lead to significant loss of sensation, muscle weakness, or impaired movement. Surgical repair aims to restore nerve continuity, allowing for the gradual return of function and sensation.

  • Radial Nerve – Provides sensation to the back of the hand, the thumb, and the back of the index finger. It also helps extend the wrist and fingers.
  • Ulnar Nerve – Provides sensation to the little finger, half of the ring finger, and the inner (ulnar) side of the palm. It also controls fine motor movements of the hand.
  • Median Nerve – Provides sensation to the middle finger, half of the index and ring fingers, and the outer (radial) side of the palm. It plays a key role in thumb and finger flexion.

  • Neuropraxia – A mild injury where the nerve is stretched but remains structurally intact. There is temporary sensory or motor dysfunction, often with full recovery.
  • Axonotmesis – A moderate injury where the internal nerve fibers are damaged while the outer sheath remains intact. This causes temporary loss of function with a longer recovery period.
  • Neurotmesis – The most severe form, in which the nerve is completely severed, resulting in a total loss of motor and sensory function. Surgical intervention is necessary.

  • End-to-End Repair – The two ends of the severed nerve are directly sutured together.
  • Nerve Graft – A segment of another nerve (usually taken from the patient’s leg) is used to bridge the gap in the damaged nerve.
  • Vascularized Nerve Graft – Similar to a nerve graft but includes its own blood supply, used in cases with compromised circulation, such as burns.
  • Nerve Transfer – A healthy nerve is rerouted to restore function in the area of the damaged nerve.

Due to the complex structure and function of hand nerves, early diagnosis and appropriate surgical repair, followed by targeted rehabilitation, are essential for optimizing recovery of both sensory and motor function.

Nerve injuries in the hand can lead to loss of sensation, function, or both. While some nerve injuries may heal on their own, others require surgical intervention. Timing for nerve repair depends on the nature of the injury:
Complex injuries: Nerve repair is typically done 3 to 6 weeks after the injury to allow better outcomes.
Isolated nerve injuries: Surgery may be performed sooner to increase the chances of full recovery.

If a nerve is cut or severed, it may be reconnected to its other end. In cases where this isn’t possible, a nerve graft may be performed, using a healthy nerve taken from another part of the body to bridge the gap.

The specifics of nerve repair surgery vary depending on the type of repair—direct repair, nerve graft, or nerve transfer—as well as the location and severity of the injury.

In most cases, the procedure is performed under general anesthesia and may take between 1 -3 hours.
